We are seeking an energetic, creative, compassionate, organized and flexible person to join our team to provide case management services to our guest families.
The primary responsibility of the Case Manager is assuring guest families receive the tools necessary to become self-sufficient and ultimately move into and remain in affordable, sustainable housing.
Essential Functions:
Provide weekly case management to guest families in the shelter program & those in Prevention/Diversion program.
Conduct telephone and in-person intake interviews with prospective families and make recommendation to Executive Director those families meeting requirements to move into our program.
Develop family case plans; meet with guests to monitor progress in securing housing, employment, job training and childcare; assist guests with social service needs and referrals to other organizations as appropriate; advocate with agencies for guests as needed under the direction of the Executive Director.
Develop and follow-up on family goals and progress.
Maintain written documentation of all visits, goals and updates.
Serve as point of contact with guest families and congregation Coordinators.
This may require after hours responses to emergencies.
Develop and maintain relationships with local landlords and assist families with housing options.
Document all case management meetings and complete weekly reports.
Perform routine office/clerical duties as required.
Comply with all policies and procedures of Family Promise of Clear Creek Perform other associated duties as assigned by Executive Director.
Coordinate congregation rotation for guest families.
Lead monthly coordinators meetings, and others.
Attend Life Skills classes on Tuesday evenings and Saturday afternoons at least once every other month.
Monitor guest families to ensure the Day Center is appropriately maintained.
Maintain updated resource and referral list.
Manage, schedule and organize volunteers at the Day Center.
